DFSV GP Action Plan and Referral
The Domestic, Family, and Sexual Violence (DFSV) Action Plan's primary focus is to provide a tool to guide GPs in patient risk assessment, escalation, safety planning and development of a patient plan to reduce risk of harm, provide patients supports and to structure ongoing follow-up, so that patients can live free from abuse and violence.
Completing the Action Plan
The DFSV Action Plan will assist in spotting the signs, starting the conversation, recording for safety, and link for support. This doubles as the referral to your Care and Connect Linker and can be completed over several appointments, when patient's safety is at risk. Partially completed is acceptable when referring to the Care and Connect Linker for support early.
The DFSV GP Action Plan can be imported and integrated within Best Practice and Medical Director software and sent via Medical Objects.
The GP Action Plan that was developed by Hunter New England and Central Coast Primary Health Network has been updated with support from the Nepean Blue Mountains and Central and Eastern Sydney PHNs.
