  Immunisation service providers are required to use this checklist to carry out a self-audit at least once every 12 months, and more frequently if there have been problems with equipment or cold chain breaches. Please discuss any concerns or issues with your GP Support Officer.
• Strive for 5: Minimum/maximum vaccine refrigerator temperature chart:
  This is the only temperature logging chart that is approved for use by the Public Health Units. It is important that it is printed in colour. You can order this resource to be sent out to you through National Mailing and Marketing (see information on website via link provided).
