The Department of Health and Aged Care established the My Aged Care website to help you navigate the aged care system. If you need some help, the My Aged Care team can answer most of your questions over the phone. For further question please call 1800 200 422.
MyHealthConnector Directory
The My Health Connector website is a free online directory to help people improve their social connections and lifestyle supports. The directory provides information about local health and community services available to support people living in our region. My Health Connector can be used by both services and community members. It can also be used as a resource for Health connectors and Community connectors to promote services available in the Blue Mountains, Hawkesbury, Lithgow and Penrith areas.
Aged Care Programs
- Connectors now available to support social connections for older people.
- Dementia care information and support including upcoming events.
- Intergenerational Programs bring together older adults from the community and residents of independent living with children aged between 3 and 5 years for group-based interactions.
- Care finders support vulnerable older people who do not have family or friends who can help, and who would not be able to arrange services without intensive support.
- Mobile Occupational Therapy service for older adults living at home in Lithgow.
Palliative Care Resources
Palliative care is so much more then end-of-life care. It is treatment of the discomfort, symptoms and stress for people of any age who have a life-limiting illness. For more information and resources, visit our Palliative Care page.
Advanced care planning involves planning for your future, and making decisions now about the healthcare you would or would not want to receive if you became seriously ill and unable to communicate your preferences or make treatment decisions. Falls Prevention Information
Falls are the leading cause of accidental injury in older people and the leading cause of injury-related deaths in Australia. In Australia, 30% of adults aged 65 and over will experience at least one fall a year. In NSW, around 27,000 falls are severe enough to need hospitalisation every year.
