Healthy Ageing Quality Improvement - Mobility Program
Primary Health Networks (PHNs) received funding to support the Australian Government’s response to the Royal Commission into Aged Care Quality and Safety. The aim of the Healthy Ageing Quality Improvement Collaborative is to support older adults to live at home for longer through the commissioning of early intervention initiatives that promote healthy ageing, support the ongoing management of chronic conditions, reducing functional decline.
We worked with Dr Paresh Dawda from Prestantia Health and an Expert Reference Panel, consisting of primary care and allied health professionals to determine the measures of the Collaborative.
We are now offering a Healthy Ageing Quality Improvement (QI) Program which is a short-term intensive QI program focused on improving the mobility of older patients in your practice. This is a good opportunity for practices who have not participated in a QI program in the past and would like to work on a focused project with their practice team.
General Practice Healthy Ageing QI – Mobility Program
Healthy Ageing QI - Mobility Program aims to support general practice in improving the management of patients with frailty or mobility issues. The program will assist practices to implement important preventative care measures to minimise risk of hospitalisation of older people. Patients are encouraged to be equal partners with their care team to better manage their strength and balance to avoid preventable hospitalisation.
At least one GP and one Practice Nurse will be required to attend an online orientation, regular in practice meetings with the project lead and an online wrap up session to share their experience of the program with other participating practices.
Benefits to your practice
- Participation and outcome payments for the practice (up to $3,000 excluding GST).
- Increased awareness of strength and mobility supports available for your patients.
- Contributes to QI requirements for both accreditation and PIPQI.
Eligibility
During the Preparation Phase of the program, practices recruit and maintain a register of 10 eligible patients to participate in the program. Patients eligible for the program need to:
- be older patients (65 years or over and 55 years or over for Aboriginal Torres Strait Islander peoples)
- have one or more frailty indicators or frailty flag
- be likely to benefit from participation in the program to reduce their risk of hospitalisation
- be living independently, not in residential aged care.
